Thank you for your interest in the Summer Institute for Medical Students program. Before you begin this application, please be sure to gather all required information including your response to the essay question and your Curriculum Vitae. Application questions may take an hour or more to complete and a partially completed application cannot be saved for later. Incomplete applications will not be submitted and candidates may not apply more than once per year.

In addition to completing the form on this page, two letters of recommendation are required. Please see "Upload your Letters of Recommendation" for instructions.

Please note that we will begin to fill the limited number of program seats available immediately following the PRIORITY application deadline on January 31, 2012 at midnight, Pacific time. To be considered for a remaining seat, if any, applications will be accepted until March 4, 2012 at midnight, Pacific time. No applications will be accepted after the final deadline.

Applicants and alternates invited to participate in the program will be notified by e-mail by March 31, 2012. Applicants who are not selected will be notified by April 6, 2012.

*Letters of recommendation should be addressed to "SIMS Program Director" and describe why you are a good candidate to participate in the SIMS program. If the person writing the letter wants to send it under separate cover, please be sure your full name and date of birth are referenced in the letter (so we may match it to your application).
